Ileocecal valve problem. How to differentiate with a grumbling appendix. Referred pain from an organ dysfunction.

Question:
Hi..heard you on radio today..most interested in your remarks re referred pain..I have scoliosis..prolapsed discs L4/5.....and upper discs mid back have previously spontaneously fractured in the past. However - I live with this..but for the past year have had severe intermittent right sided abdominal pain.(level with navel - half way between navel and side. It is stitch like - ct scans ultra sound ,gastrascope etc..have not found the problem..do you think the pain could be associated with my back problems (I do not eat nuts..allergic to quite a few foods..so careful with what I eat)..."Sarah"

Answer:
Hi "Sarah",
I hope you have had the pain you describe in the right side of your abdomen sorted out. It could well be an irritated ileocecal valve. This is a small ring of muscle situated between your small and large intestine, just above the location of the appendix. It allows food to go through one way only. Sometimes it can get irritated by food that is too rough for it, and there can also be a back up of food in the small intestine which can cause pain and bloating in the abdominal area.

In the same location, you could have an irritated appendix. This often causes pain on touch around the navel itself which doesn't happen as much with the ileocecal valve problem so can be used as a point of differentiation between the two conditions.
If you have an irritated ileocecal valve, this can cause referred pain to your back, not so much at the level of L4-L5 but higher up at the level of L1 or lower down at S1 which is the first part of the sacrum. The kind of low back pain you feel from this is a constant dull ache that no change of movement relieves. It can get worse and then feel better with seemingly no reason. The reason for the changes in pain intensity is the referred pain is responding to how irritated the ileocecal valve is feeling.
Dietary changes are the main thing you can do to help the ileocecal valve. I can tell you more about that if you are interested .
